Why is this needed?
Every year, millions of Europeans are affected by stroke. Whether returning home from a hospital or rehabilitation centre, the transition can be a challenging phase of recovery. Patients must continue their rehabilitation with less direct professional support while adapting to everyday life at home.
Research shows that the transition from hospital or rehabilitation to home is one of the most challenging phases of recovery.
Poor continuity of care
Patients often leave hospital without sufficient support.
Low activity after stroke
Many patients become increasingly sedentary during recovery.
Caregiver burden
Families and clinicians face growing demands.
Healthcare workforce shortages
Healthcare systems are under pressure.
Lack of connected rehabilitation technology
Few tools support patients across the entire care pathway.
A clinical trial to collect evidence
A clinical trial will be conducted with Bispebjerg and Frederiksberg University Hospital in Denmark and Roessingh, Centre for Rehabilitation in The Netherlands. A total number of 188 stroke patients will participate.
