AI-Rehabilitation Companion

A more responsive path through cardiac rehabilitation.

TimelineJan 2026 — Jun 2027
ConnectedCare’s roleConnectedCare leads the project and contributes Lizz.Health, conversational interaction and patient-centred development.

Cardiovascular rehabilitation often relies on infrequent assessments. This cross-border project connects ConnectedCare’s Lizz.Health companion with InterMedCon’s LifeLeaf smartwatch, helping patients and clinicians stay in touch with daily progress.

An AI layer brings wearable measurements and patient-reported outcomes together in a FHIR-based health model. It selects timely, relevant conversations — from activity encouragement to dietary guidance or escalation to a clinician — while keeping language anchored to validated medical content.

The team is developing a TRL 7 prototype iteratively with patients and healthcare professionals in the Netherlands and Germany.
