E-SOM
Better signals for medication review.
Medication-related hospital admissions are largely preventable: in the Netherlands alone, around 19,000 admissions per year could be avoided. Older patients using five or more medications chronically are at highest risk, but selecting the right patients for a medication review has proven difficult in practice.
E-SOM developed an eHealth application enabling older patients to self-report health signals from home. It combined a patient and caregiver module for self-reporting with a pharmacy module that gives pharmacists an overview of patient data to support clinical decision-making.
The application was built through an iterative co-design process in sprints with patients, caregivers and pharmacists, and validated through heuristic testing and a feasibility pilot in community pharmacy settings.
