Lizz-TRANSCARE
Continuity of care, from clinic to home.
Leaving a hospital or rehabilitation centre is a critical moment in stroke recovery. Lizz-TRANSCARE combines ConnectedCare’s Lizz social companion with SENS motion sensing to keep rehabilitation goals part of everyday life.
Lizz provides personalised coaching, medication reminders and companionship. Patients first meet the companion in the clinic and take it home at discharge; SENS helps care professionals monitor physical activity remotely and adjust therapy.
Two parallel randomised controlled trials in Denmark and the Netherlands will involve 188 participants and evaluate sedentary behaviour, quality of life, daily living, mood and self-efficacy.
