When one of your parents, your partner or you are diagnosed with dementia, you might have many questions. ‘What are things I should think about, what should I plan and organize? To whom can I turn to for help?’. Currently there is a lot of information available for people with dementia, their relatives and their informal caregiver. However, the amount of information is overwhelming and people that are facing dementia have difficulty finding the information that could help them at that moment. Through research with the other partners, end-users and knowledge institutions, we have created that fits the target group. With Dementiewijzer Delft, we have collected all the information that is relevant for people living in Delft. On the website www.dementiewijzerdelft.nl, you can find the details about support that is provided for people with dementia and their caregivers in Delft. With Dementiewijzer Delft, the collaboration between informal and formal professional (care) institutions in the region has strengthened. Moreover, the citizens can find the information they need step-by-step, leading to a less overwhelming experience with (caring for someone with) dementia.
